How long does it take before an opt out is effective?

Immediately.

Within a few seconds the webhooks should trigger, and any further attempts to contact that phone number will come with a rejection code#1

Are opt outs for the account for the whole system?

Opt outs are specific to each account.

How do I get my opt outs?

You can use the Export Data feature to download a historical export of all your opt outs. This is good for adhoc syncing of your list.

For real time syncing, you can leverage the Webhooks and subscribe to the "opt out" events
